1. Important Notice – Risks of Physical Activity

Chokdee Martial Fitness Academy provides physically demanding activities including Muay Thai, boxing, Brazilian Jiu-Jitsu, wrestling, MMA, sparring, strength and conditioning, fitness training and use of gym equipment. These activities carry inherent risks that cannot be completely eliminated.

Possible risks include, but are not limited to: bruising, cuts, grazes, sprains, strains, muscle soreness, joint injuries, dislocations, fractures, concussion, head injury, dental injury, eye injury, internal injury, illness, aggravation of an existing condition and, in rare circumstances, serious injury or death.

3. Medical, Health & Fitness Declaration

I confirm that, to the best of my knowledge, I am fit to participate in the activities I have chosen. I will inform Chokdee Martial Fitness Academy of any medical condition, injury, medication, recent surgery or other circumstance that may affect my ability to participate safely.

If staff reasonably believe that participation may be unsafe, they may ask the participant to stop, modify an activity or seek appropriate medical advice before returning. This does not replace professional medical advice.

5. Martial Arts, Contact Training & Sparring

Where contact training or sparring is permitted, participants must follow the coach's instructions and the rules for the specific class, age group and experience level.

6. Use of Gym Equipment & Facilities

Participants are responsible for using equipment safely and only as instructed. Equipment must not be used for purposes for which it is not intended.

8. Injuries, First Aid & Emergency Procedures

I understand that accidents and injuries can occur during physical activity. First aid may be provided by a trained member of staff where appropriate. Where an injury appears serious or urgent, emergency medical services may be contacted.

10. Personal Belongings & Property

Participants should avoid bringing valuables into the gym. To the extent permitted by law, Chokdee Martial Fitness Academy is not responsible for theft, loss or damage to personal belongings except where liability cannot lawfully be excluded.

Participation is undertaken with an understanding that physical activity and contact sports carry inherent risks. Nothing in this document is intended to exclude or restrict any liability that cannot lawfully be excluded or restricted, including liability for death or personal injury caused by negligence.

Nothing in this document removes or limits any statutory rights that apply to the participant. Terms relating to consumers must be fair and transparent. This document should therefore be read alongside the gym's membership terms, policies and any applicable booking or cancellation terms.

13. Membership, Bookings & Cancellation

Memberships are subject to a minimum three-month cancellation period. Requests to cancel a membership must be made in accordance with the gym's current cancellation procedure, and membership payments remain due during the applicable three-month cancellation period. For 1-to-1 coaching sessions, cancellations or missed appointments made with less than 24 hours' notice are charged at the full session price and are payable to the coach.

I confirm that I have read and understood this document. I have had the opportunity to ask questions about the activities, risks and rules. I understand that participation involves inherent risks and agree to follow the gym's safety rules and instructions. I understand that this document does not remove any legal rights or liabilities that cannot lawfully be excluded.

Online Conduct & Social Media

Participants and parents/guardians are expected to behave responsibly and respectfully online and on social media. They must not post, share or publish content about the gym, its members, coaches, staff or training activities that is threatening, abusive, harassing, discriminatory, defamatory, deliberately misleading, or otherwise inappropriate. Participants must not use the gym's name, logo, images or the names/images of coaches or other members in a way that could reasonably damage the reputation of the gym, its coaches, staff or members. Private disagreements or complaints should be raised directly with the gym so they can be addressed appropriately. Nothing in this section prevents a person from making a lawful complaint, reporting a safeguarding or safety concern, or exercising any legal right.

Memberships & Separate Strength & Conditioning Facility

Memberships cover access to the martial arts classes included within the member's chosen membership. The Strength & Conditioning Gym is a separate, private training area and is not included in a standard martial arts membership unless specifically stated in the member's membership agreement. Access to the Strength & Conditioning Gym is an additional service and may require a separate membership, session booking or additional payment. The Strength & Conditioning area may only be used when a session has been booked or when the member is training with an authorised coach. Members must not enter or use the Strength & Conditioning area independently or without the required booking, supervision or authorisation.

9. Photography, Video & Promotional Use

Photography and video may sometimes take place at the gym for training records, events or promotional purposes. Promotional consent is optional and separate from permission to participate.
