I confirm that I am the parent or legal guardian of the young person named above and have authority to give consent for them to participate in activities provided by Chokdee Martial Fitness Academy.

4. Risks of Participation

I understand that physical activity and martial arts can result in injury even when reasonable care and safety procedures are followed. Possible injuries include, but are not limited to, bruises, cuts, grazes, sprains, strains, muscle soreness, joint injuries, dislocations, fractures, concussion, head injury, dental injury, eye injury and, in rare circumstances, serious injury or death.

Nothing in this form is intended to exclude or restrict any liability that cannot lawfully be excluded or restricted, including liability for death or personal injury caused by negligence.

5. Medical & Health Information

6. Emergency Medical Consent

If my child becomes injured or unwell during a session, I authorise staff to provide appropriate first aid within their training and to contact emergency services where reasonably necessary. Staff will attempt to contact me or the emergency contact as soon as reasonably practicable.

7. Martial Arts, Contact Training & Sparring

Contact training or sparring will only take place when authorised by the coach and in accordance with the rules, age/ability group and protective equipment requirements of the session.

8. Gym Equipment & Strength/Conditioning

9. Behaviour, Respect & Safeguarding

10. Collection & Supervision

The parent/guardian should ensure that the gym has accurate contact and collection information. Unless separately agreed in writing, the gym's responsibility relates to the agreed class/session and does not extend to unsupervised time before or after it.

12. Personal Belongings

Valuable personal belongings should not be brought into the gym. To the extent permitted by law, the gym is not responsible for loss, theft or damage except where liability cannot lawfully be excluded.

13. Membership, Bookings & Cancellation

Memberships are subject to a minimum three-month cancellation period. Payments remain due during that period. For 1-to-1 coaching, cancellations or missed appointments with less than 24 hours' notice are charged at the full session price.

14. Parent / Guardian Declaration

I confirm that I have read and understood this form, had the opportunity to ask questions and understand the activities and their inherent risks. I confirm that the information provided is accurate and that I will update the gym if circumstances change.

Online Conduct & Social Media

Participants and parents/guardians are expected to behave responsibly and respectfully online and on social media. They must not post, share or publish content about the gym, its members, coaches, staff or training activities that is threatening, abusive, harassing, discriminatory, defamatory, deliberately misleading, or otherwise inappropriate. Participants must not use the gym's name, logo, images or the names/images of coaches or other members in a way that could reasonably damage the reputation of the gym, its coaches, staff or members. Private disagreements or complaints should be raised directly with the gym so they can be addressed appropriately. Nothing in this section prevents a person from making a lawful complaint, reporting a safeguarding or safety concern, or exercising any legal right.

Memberships & Separate Strength & Conditioning Facility

Memberships cover access to the martial arts classes included within the member's chosen membership. The Strength & Conditioning Gym is a separate, private training area and is not included in a standard martial arts membership unless specifically stated in the member's membership agreement. Access to the Strength & Conditioning Gym is an additional service and may require a separate membership, session booking or additional payment. The Strength & Conditioning area may only be used when a session has been booked or when the member is training with an authorised coach. Members must not enter or use the Strength & Conditioning area independently or without the required booking, supervision or authorisation.

11. Photography & Video Consent

Photography and video may sometimes take place at the gym for training records, events or promotional purposes. Promotional consent is optional and separate from permission to participate.
